I agree to the following terms and regulations with regards to the request:

  1. Ownership. The Softphone ID assigned to you is owned by the companyand issued to you for the duration of your employment and/or assignment with the company.
  2. Delivery. The Softphone ID will be issued to you via email within 24-48 hours of your request. Included with the Softphone ID will be instructed on setting up your headset and your Softphone ID with the VCC software.
  3. Returns. Once your employment is terminated with the company, or you no longer require the need to use your Softphone ID, the Softphone ID will be returned to inventory and assigned to another team member.
  4. Support. The Softphone software and ID, its functionality and connectivity is gauranteed by the company. In the event the software or setup requires support, SUPDEV will provide assistance, as necessary, to assist the employee. This support is provided at no additional charge.
  5. Employment. I also agree that I am gainfully employed or have been hired by the company in which I am requesting an ID and my employment can be verified upon request.
